After School Programs

After school programs serve children and youth grades K-12. These programs focus on academic support, mentoring, create new friendships, improve overall mental and physical health and well-being. That's why Peach State Health Plan members get one voucher per year for after school activities, youth club memberships and sports.

To get this VAB, please complete the form below. Legal guardian must complete form on behalf of member. Eligibility Statement: Eligibility requirements apply. One voucher per child every 12 months. Must have completed an annual well visit and Dentist visit within the last 12 months. Must complete separate form for each child.

Girl Scouts® Membership

Covers annual membership fee for grades K-12.

  • Includes supply fee for badges and patches.

Qualification Requirement: Must have visited their PCP & Dental starting  July 1, 2022.

Boys & Girls Clubs® Membership

Covers up to $60 of annual membership fee for grades K-12.

  • Enjoy health plan programs and fun activities during the school year at participating clubs.

Qualification Requirement:

  • Must have visited their PCP & Dental starting  July 1, 2022

ATL Metro Boys & Girls Clubs are not in network. A $60 Gift card will be mailed to parents (for eligible members) to redeem at identified ATL METRO CLUBS listed. The member must have visited their PCP and Dentist within the last 12 months. Peach State does not pay reimbursement fees on Value Added Benefits.

How to access Benefits:

Registration between July 1, 2021 through April 1, 2023.

Sports Scholarship

Youth Team Sports participants ages 6-18 can earn up to $75 for uniforms and registration fees.

  • Available at participating Parks and Recreation Centers, YMCA, and other select facilities.
  • Sports include football, soccer, basketball, cheerleading or swimming lessons.
  • One sport choice per year, per member.

Qualification Requirement:

  • Note: Swimming lessons (6-8 lessons) benefits covered at participating YMCAs only with a pool.
  • Must have visited their PCP & Dental starting  July 1, 2022.

How to access Benefits:

Registration between July 1, 2021 through April 1, 2023.

Peach State Health Plan does not cover Summer Camp

AFTER SCHOOL PROGRAM FORM

You must have a Medicaid ID in order to register; if you do not know your Medicaid ID number; please call the Member Services to register at 1-800-704-1484.

Choose which After School Program you want: You may only register for 2 Value added benefits, annually.

We Do Not Cover Summer Camps. Please note that partners may be added/removed based on contracting.
