Hypoallergenic Bedding
Members with asthma can receive hypoallergenic bedding with mattress and pillow casings to help control asthma triggers.
Qualification Requirements:
- Eligible PSHP members with a diagnosis of asthma
- Limit of $100 annually
- Requires Care Manager approval

Free Memberships
You can choose one free annual membership to Sam’s Club, Costco, BJ’s, or Amazon.
Qualification Requirements:
- Quality health activities must be completed after July 1st, 2023.
- Age 21-49 (Female): Complete Cervical Cancer Screening
- Age 50-75 (Female): Complete Breast Cancer Screening
- Age 21-44 (Male): Complete Dental Visit
- Age 45-74 (Male): Complete Colorectal Cancer Screening
- One membership per calendar year
- Members must present the gift card within the current benefit year (July 1st, 2024, through June 30th, 2025)
