Aging Out of Medicaid Coverage

Did you know you have to apply for Medicaid when you turn 19?
You must apply for Georgia Medicaid benefits once you turn 19. If you don't apply for Medicaid on time to confirm your Medicaid eligibility, you may lose your Medicaid benefits.
Why is this important?
These benefits may include behavioral and mental health support, transportation and housing support or continuing education opportunities.
Applying for Medicaid:
- You should receive a letter from the State of Georgia in the mail stating that you need to apply for . Open all mail from the State of Georgia and take action right away.
- Gather your documents. Some examples of the documents you may need include:
- Social Security Card
- Paystub
- Most recent W-2 form
- Utility bill
- Fill out the Medicaid Assistance Application completely. Take your time and answer all the questions.
- Submit the form, including any required documents.You can submit the application online, by mail, by phone or in person.
- Apply Online
- You can apply online through Georgia Gateway.
- Sign in with your login information.
- Fill out all of the required information and submit your form.
- Apply by Mail

- Mail the completed form to your local DFCS office.
- Apply by Phone
